Abstract:

Background and Objectives: Most of patients with multiple sclerosis (MS) (even those with mild degrees) experience various sexual disorders, which can affect their different aspects of life. Therefore, it is important to find treatments for them. This study aims to assess the effectiveness of mindfulness-based intervention and neurofeedback on the sexual function of women with MS. Methods: This is a quasi-experimental study with a pre-test/post-test/follow-up design. The study population includes all married women with MS in Karaj, Iran in 2020, 16 of whom were selected using a purposive sampling method and were randomly divided into two experimental groups of neurofeedback (n=8) and mindfulness (n=8). To collect data, the female sexual dysfunctional inventory by Naderi et al. (2011) was used. The collected data were analyzed in SPSS software, version 26 using repeated measures analysis of variance. Results: According to the results, both mindfulness-based intervention and neurofeedback were effective in improving the sexual functions of women with MS, and there was no significant difference between the two groups (P<0.05). Conclusion: Both Mindfulness-based intervention and neurofeedback can improve the sexual functions of women with MS. They can be used as an effective complementary treatment in MS women.

Purpose: To report a patient with MEWDS.Patient and findings: A 65-year-old man presented with sudden decrease of vision in his right eye. On ophthalmoscopy of the involved eye, numerous small discrete white lesions were detected superficially and deep in the retina around the disc and macula. On visual field examination, enlargement of the blind spot was detected. Fluorescein angiography was unremarkable. On electroretinography, the a-wave was decreased minimally. The patient was followed and after 2 months, significant improvement of visual acuity and visual field was observed. Conclusion: MEWDS is a rare intraocular inflammatory disease of unknown etiology and in contrast to our patient, more frequent in females. The natural course is generally benign with spontaneous disappearance of the lesions and improvement of vision in 6 to 8 weeks.

Abstract:

Purpose: To report a case of severe macular edema following the taking of fingolimod in multiple sclerosis.Case Report: A 34-year-old woman with a long history of relapsing-remitting multiple sclerosis presented complaining of blurred vision in the left eye that started 1 week before the presentation. She had been taking fingolimod for 4 months. She was not diabetic and had no history of uveitis but reported an episode of retrobulbar optic neuritis in both eyes. The initial ophthalmic examination, which was performed 2 weeks before initiating fingolimod, revealed a best corrected visual acuity (BCVA) of 20.25 OU and no ocular abnormalities. Upon the recent presentation, BCVA was 20.25 OD and 20.70 OS and there was no afferentpapillary defect. The examination of anterior segment and anterior vitreous was unremarkable. Retinal examination of the left eye revealed cystoid macular edema with no other inflammatory signs or vascular changes. The findings of spectral domain optical coherence tomography (SD-OCT) were compatible with macular edema and indicated that the central macular thickness (CMT) was 605 microns. Fingolimod was discontinued and sinarolac was started three times daily. After 6 weeks, BCVA was 20.25 OD and 20.30 OS.Macular edema was resolved clinically and SD-OCT exhibited a significant reduction in macular edema with a CMT of 277 microns. Conclusion: Fingolimod-associated macular edema is a benign complication of the drug used for multiple sclerosis and. This complication responds well to the drug discontinuation and topical non-steroidal antiinflammatory administration.

Abstract:

Introduction: Myocardial bridging is a congenital condition in which a segment of a major epicardial coronary artery has an intra myocardial route. Myocardial bridging is usually confined to a single vessel and is typically the mid segment of the left anterior descending artery. Bridging may be associated with chest pain, myocardial infarction, and sudden cardiac death, or usually asymptomatic.Case Report: A 59 year-old women with diabetes mellitus and hypertension presented with dyspnea and chest pain on exercise. Physical exam, electrocardiography and echocardiography were intact. In angiography, we found severe stenosis of left main coronary artery and severe stenosis of three coronary artery (Three Vessel Disease) as well as multiple myocardial bridges at the end of right coronary artery. Conclusion: Myocardial bridging is a rave condition, however, it be considered in patients facing recurrent chest pain, it has severe complication such as sudden cardiac death.

Abstract:

Introduction: Uterine fibroids (leiomyomas) are the most common benign tumors of the female reproductive system, affecting approximately 20% of women of reproductive age. These tumors can be single or multiple, and depending on their size and location, they may be asymptomatic or present with symptoms such as abnormal uterine bleeding, pelvic pain, infertility, or pressure effects on adjacent organs. In symptomatic cases, especially when fertility preservation is desired, accurate diagnosis and individualized treatment planning are essential. Case Presentation: A 42-year-old woman with a history of two cesarean deliveries and symptoms of abnormal uterine bleeding including prolonged heavy menstruation with clot passage was referred to Sarem Women’s Hospital. Imaging revealed multiple myomas of varying types and locations, including a large submucosal fibroid extending into the endometrial cavity, intramural myomas in both anterior and posterior uterine walls, and hypoechoic intracavitary structures suggestive of additional myomas. Nabothian cysts and small endometrial polyps were also identified. The final diagnosis was intracavitary myoma, and the patient underwent hysteroscopic myomectomy and polypectomy. Conclusion: The successful management of this patient using minimally invasive hysteroscopic surgery highlights the importance of precise evaluation of fibroid type, size, and location in determining the most appropriate treatment approach. Advanced imaging techniques, such as Saline Infusion Sonohysterography (SIS), combined with comprehensive clinical assessment, play a pivotal role in surgical planning, particularly for women seeking to preserve their uterus and fertility potential.

Abstract:

Background: microRNAs are non-coding RNAs that modulate different types of cellular processes like differentiation and cell death. Hitherto several studies have been done to determine the expression profile of microRNAs in patients with multiple sclerosis (MS) to obtain appropriate biomarkers. In previous studies, it was found that miR-145 was over-expressed. This up-regulation was in patients who did not start taking medicine. Therefore, in this study we assessed the effect of beta interferon on the expression of this microRNA in patients with multiple sclerosis. Methods: We evaluated the expression pattern of miR-145 in 15 patients who did not start taking medicine and called treatment naive، in 15 patients who were under treatment and also 15 healthy people using real-time polymerase chain reaction method.Findings: The expression level of miR-145 in treatment naive patients was 3.9 fold of healthy people (P=0.005), whereas expression level of this microRNA between healthy people and under treatment patients was not significantly different. Conclusion: Down-regulation of miR-145 in under-treatment patients to the extent of healthy people suggests that probably, this microRNA could be used as predictive biomarker.
